GPAQ Individual Level

This survey format provides personalised reports for individuals in the practice, and is therefore the ideal solution for GP Trainers and trainees, or GPs and nurses wishing to collect data for revalidation folders.

Nurses and nurse practitioners have their own questionnaire design.

How it works

  • Individual kits comprising 100 questionnaire forms and desktop dispensers are allocated to each member of the practice taking part as individuals. 
  • Questionnaires are given to all patients of the practice (not temporary residents) and at least 16 years old attending the surgery from the day the survey starts until all the questionnaires have been given out.
  • Patients complete the questionnaire after their consultation, and return the completed surveys via the postage-paid reply service to PatientDynamics. This option offers a higher degree of anonymity to the patients taking part.
  • Surgery posters and further information packs are also available to publicise the survey and the results process to the patients.
  • We monitor the returned questionnaires and start processing the data (data entry, cleaning and validation) when enough questionnaires have been returned. For individual surveys the emphasis is on higher response rates (at least 50 per doctor) so that more robust reports and comparisons can be made.
  • We send you reports within one week of completing the fieldwork. PatientDynamics will incorporate practice and individual data together for the practice report. For an example, click here.
