PatientDynamics is pleased to offer you a bespoke online survey (up to 40 questions) and report from £200 +vat

Methodology

Send us your questions and we will create the online and the hardcopy questionnaire. You can then send an email to your patients with the link to the online survey or print the questionnaires out and give them to your patients. Return them to us for collating, processing and reporting.

Simple.

Both methods work and the data from each method are combined in to the same survey report.

Survey Price (all prices subject to vat)

• £200 questionnaire design
• production of online and paper questionnaire master
• Optional £0.48p per copy (A3 double sided) printed by PatientDynamics with £10 postage and packing
• £0.49 per responder / questionnaire processed (paper)
• £50 for reporting (in web & paper format)
